Case Manager
Milton Diaz joined Project Guardianship in September of 2022. Milton graduated from Hunter College with a Bachelor’s Degree in Psychology & Human Services. Since then, Milton has dedicated his career to providing person-centered care to historically underserved populations.
Milton has worked as an intake coordinator at the NYC Anti Violence Project, referring clients in vulnerable situations to supportive services. He has worked with homeless youths and adults with severe domestic violence experience, intellectual disabilities, and physical disabilities as a case manager at Westhab Inc. Here, he conducted assessments, performed psychosocial evaluations, assisted clients with accessing community-based supports and mental health services, and implemented service plans. Milton has also worked as an esthetician and makeup artist while managing a cosmetics shop.
